Caregiver Education Through the Implementation of PATH-s, Caregiver Education Addressed with an Inpatient Population Utilizing the PATH-s and Occupation-Based Interventions.

This course is over caregiver education through the implementation of an evidence-based caregiver assessment. Related literature through a needs assessment and critically appraised topic are provided to support the need for increased education prior to individuals discharging home from an inpatient rehabilitation hospital setting. The Canadian Model of Occupational Performance (CMOP) as well as the Person Environment Occupation Performance (PEOP) model guide this doctoral capstone project with an emphasis on program development. The caregiver assessment utilized is the Preparedness Assessment for the Transition Home after Stroke (PATH-s). This assessment assesses gaps in caregiver education to ensure patients and caregivers are discharged with the tools they need to be successful. The assessment was not limited to stroke patients as it was utilized with a variety of patients at the inpatient hospital where this capstone experience took place. The gaps in caregiver education are addressed through occupation-based interventions which were created specifically for each item on the PATH-s. The caregivers are also given additional resources, which are included in this course, such as handouts to address daily occupations and caregiver burnout. Caregiver data shows implementation effectiveness and caregiver comfortability approximately 10 days post patient discharge. Quiz questions are provided for learners to test their knowledge about the course.
